Our Areas of Expertise
Our 5 individual partners have specific interests in the specialist fields of vascular, colorectal, hepatobiliary, pancreas, hernia, thyroid and breast surgery. Our operating theatres are equipped with cutting edge equipment to perform complex endovascular and laparoscopic procedures. We also perform upper and lower gastro-intestinal endoscopy in our rooms assisted by our qualified nursing staff.
Laparoscopic Surgery
Minimally invasive abdominal & pelvic surgery
While all partners are experienced in laparoscopic surgery several have concentrated on this skill which has transformed the way we approach modern surgery. These techniques have revolutionised the management of gallstones, gastro-oesophageal reflux, inguinal and incisional hernia. They are also applicable to the surgery of achalasia, splenectomy, adrenalectomy and sympathectomy for hyperhidrosis. Two of us are trained in, and have substantial experience in laparoscopic colorectal surgery. We have over 300 patients in our colorectal case series.
Hepatobiliary / Pancreatic Surgery
Surgery of the liver, pancreas, bile ducts and gallbladder.
An ERCP service, both diagnostic and interventional, is provided. Bile duct stones are removed, obstucted ducts drained and malignant strictures stented. Complex surgical procedures, such as liver and pancreas resections are undertaken, often in conjunction with the Hepato-biliary unit at Groote Schuur.
Colorectal & Upper Gastro-intestinal surgery
Bowel and Gastro-Intestinal Tract Surgery
In the lower GI tract, 2 partners have subspecialised in colo-rectal carcinoma, inflammatory bowel disease, complex proctology and fistula surgery, faecal incontinence, sacral nerve stimulation and transanal endoscopic surgery. The exciting new field of rectal preservation in the management of rectal cancer is a special interest of the colorectal surgeons in the practice. There is extensive experience with GI complications resulting from gynaecological disease, including cancer, infertility and endometriosis. Minor ano-rectal procedures are performed in our procedure room avoiding hospital admission in many cases. Advanced surgical techniques have resulted in new combined approaches, involving radiotherapy, to the surgical treatment of low rectal carcinomas. In the upper GI tract, we have extensive experience with oesophago-gastric carcinoma, oesophageal reflux, achalasia, complex peptic ulcer disease, gastro-intestinal bleeding. We established dedicated GIT ward in Kingsbury Hospital with nursing staff who has special training and experience in this field.
Endoscopy
Gastroscopy, colonoscopy, ERCP and a full range of therapeutic endoscopic procedures
These procedures are performed in our dedicated endoscopy suites within our consulting rooms. This provides patient convenience, avoids hospital admission and results in a very significant saving in hospital costs. Patients with advanced obstructing carcinomas of the oesophagus and colon are treated with stenting. We were the first to use Argon Plasma coagulation in the Cape and it remains a special interest area.
We have highly trained and experienced endoscopy nurses. Endoscopic examinations in hospital tend to be reserved for the occasional critically ill patient who cannot be moved. Most of the in-patients are scoped within our consulting room environment.
Arterial Surgery
Aortic and Thoracic aortic aneurysms
2 partners specialise in arterial surgery. There is a dedicated vascular theatre at Kingsbury Hospital with a state of the art fixed c-arm and full anaesthetic capabilities. This facilitates the performance of both open surgery and complex endovascular procedures. Procedures include the percutaneous treatment of abdominal and thoracic aortic aneurysms with aortic stents, carotid angioplasty and stenting and peripheral arterial angioplasty and stenting. We have vascular studies units at our Kingsbury rooms, staffed with a dedicated vascular technologist.
We have been the country’s leader in non-invasive vascular assessment for nearly 20 years. We currently employ 2 highly trained sonographers operating colour-flow duplex units. The vast majority of our vascular assessments are based on out-patient ultrasound evaluation. We seldom have to resort to expensive investigations such as CT or MR angiography and virtually never perform diagnostic angiography. These latter investigations are standard practice throughout this country because of poor access to top-quality duplex scanning.
Venous Surgery
Surgery of peripheral and central veins
Patients with recurrent varicose veins or suspected deep vein thrombosis (DVT) are assessed in the vascular studies unit. Patients with acute DVT benefit from early assessment for possible venous thrombectomy or catheter directed fibrinolytic therapy. We have an interest in the minimally invasive management of venous disease and in addition to the traditional open procedures, we also offer laser (EVLT) or radiofrequency ablation (VNUS) and ultrasound guided foam injection sclerotherapy.
Breast Surgery
Mastectomy, breast biopsy & reconstructive surgery
Since its inception, the practice has had interest in breast surgery. Close links with radiologists, oncologists, pathologists, social workers, Reach for Recover and the National Cancer Association reflect our multi disciplinary approach. Immediate and delayed reconstruction is gaining increasing acceptance and in this regard we work closely with our plastic surgical colleagues.
